Fees & Cancellation Policy:
Private pay fees are: 
$120 for assessments
$75 for individual/family therapy (50 min.)
$50 for brief counseling (20-30 min.)
$100 for extended sessions (75-80 min.) 

When billing insurance as a preferred provider, the rates are determined by the insurance carrier.

When billing insurance out of network, the fees are: 
$160 for assessments
$100 for individual/family
$75 for brief
$125 for extended.

First Choice Health Network
*Others may be covered at an out of network benefit


Billing Notes:
All payments are due at the time of service. 

There will be a full fee charge for missed or canceled appointments less than 24 hours in advance, unless the circumstance is beyond control. Please cancel your appt. if you are ill.
